By completing this registration and affixing my initials, I acknowledge and agree to the following terms for myself and all registered participants, including accompanying minors:
Assumption of Risk & Release of Liability: I understand that participating in the community block party involves inherent risks associated with outdoor gatherings, walking, uneven surfaces, crowds, weather conditions, recreational games, and food service. I voluntarily choose to participate and agree to follow all posted instructions, event rules, and directions from AKARAMA Foundation, Inc. staff, volunteers, sponsors, and vendors. I assume full responsibility for supervising any minors under my care. To the fullest extent permitted by applicable law, I release and hold harmless AKARAMA Foundation, Inc., its officers, directors, employees, volunteers, agents, sponsors, and participating vendors from any and all claims or liabilities arising from our voluntary participation.
Parent / Legal Guardian Consent: For any minor participants included in this registration, I certify that I am the parent or legal guardian, or that I have obtained full authorization from the parent/legal guardian to register them and agree to these terms on their behalf.
Photo, Video & Media Release: I grant AKARAMA Foundation, Inc. permission to photograph, film, and record me and any minors included in this registration during the event. I authorize the Foundation to use, publish, and share these images and recordings for lawful nonprofit purposes—including on its website, social media channels, newsletters, reports, promotional materials, and community outreach—without compensation or prior notice.
Medical Emergency Authorization: In the event of a medical emergency, I authorize event personnel to contact 911/emergency medical services and provide reasonable assistance until professional care is arranged. I understand event personnel are not medical providers.
Food & Activity Acknowledgment: I acknowledge that food and beverages will be available at the event and that I am solely responsible for managing any personal or family food allergies, dietary restrictions, and activity participation.
